Drama: A man amputates his penis in a psychotic episode caused by cannabis

A Thai man cut his penis with a pair of scissors during a terrifying cannabis-induced psychotic episode. The life-changing injury occurred after the unidentified 23-year-old smoked 2 grams of marijuana. the equivalent of six pipes, in two bongs.

Prior to his self-amputation, the man had stopped using cannabis for three months after being a frequent user for two years. The study was published in the prestigious Journal of Medical Case Reports website.

During the hallucination, the man became paranoid because the head of his penis seemed “distorted” as he examined himself to identify the source of the pain.

In an attempt to eliminate the problem, the man decided to use scissors to “cut” the skin from his penis before amputating it completelyleaving a two-centimeter stump and a 5-centimeter tear in the scrotum.

Incredibly, the man waited two hours before seeking medical attention in a Chiang Mai city hospital after the bleeding had not stopped. Doctors thought about reattaching the man’s penis, but it turned out to be too fragile and contaminated with ants.

And the psychosis was so intense that she experienced visual and auditory hallucinations for about two weeks during her recovery. The heartbreaking story was revealed in a medical report by the doctors who treated him.

It comes just weeks after Thailand actually decriminalized cannabis.

Describing the case in detail in the Journal of Medical Case Reports, Dr. Theerapon Tangsuwanaruk said doctors have considered implanting the amputated penis. However, he said so the man’s limb was too damaged and there was a possible risk of infection for this to continue.

“The distal penis was considered too dirty and brittle for reconstruction,” he said. “The amputated distal part of the penis was contaminated with ants and had fragile dorsal veins”, explained in detail.

After surgery to clean the wound and stop the bleeding, the man was interviewed by a psychiatrist. Urine tests performed on humans confirmed the presence of THC, the psychoactive substance in cannabis.

The psychiatrist found that the man suffered from both visual and auditory hallucinations, like seeing shadows moving and hearing animal noises such as birdsong and buzzing insects.

It was also found consistent but delusional, with a narrow emotional range. However, the professionals felt that he was not suicidal as well He was diagnosed with a substance induced psychotic disorder.

After a two-week hospital stay, the man was able to urinate while sitting and reported that the hallucinations had ceased.

Dr. Tangsuwanaruk said that a planned surgery to extend the length of the man’s remaining penis did not take place because the patient had moved.

The original source of the man’s erection pain remains a mystery, but Dr. Tangsuwanaruk he said it may have been a case of priapism, a painful erection lasting more than four hours.

However, Dr. Tangsuwanaruk said an official diagnosis of priapism in this patient’s case it is impossible due to self-amputation earlier than four hours. The NHS advises anyone with an erection that lasts longer than two hours to call 999 or go to the emergency room as it can cause permanent damage to the penis.

Cannabis-induced psychosis is a known potential side effect of drug use, although consequently autogenital mutilation is rare. Smoking cannabis can cause mild hallucinations in general, as well as a feeling of anxiety, but normally this is not psychosis.

The authors of the most recently reported case stated that while the true prevalence is unknown, there have been about 100 cases of substance-induced psychosis resulting in genital self-mutilation in the past 20 years.

However, they added that the severity of psychosis people can experience after using cannabis depends on the amount of THC consumed. Skunk, which makes up the bulk of cannabis sold on the streets of Britain, contains more THC than traditional strains.

It is deliberately created by the drug dealers with the aim of making the most potent strain of the drug possible in order to maximize their profits. Although the doctors did not specify exactly when the case occurred, Thailand is currently experiencing a cannabis revolution.

Last week it became the first Asian country to decriminalize marijuana, although its use as a recreational product remains a gray area. It followed Thailand which removed cannabis, the plant species to which both marijuana and hemp belong, from the list of illegal drugs under Thailand’s drug laws in December 2021.
